How many Royal Commissions have there been?

How many of those have produced significant, effective and lasting changes?

I thought there was a Royal Commission into the Ash Wednesday fires here in Victoria (and SA) but apparently John Cain decided against that format and it was a review. Anyway, out in the sticks it did change things and there was quite a bit of community education went on. The science moved on and further changes had to be made later but for the time it was effective in getting people prepared.
